Subpoena Submission Form for Testimony

The Office of the Medical Investigator (OMI) has developed this submission form to facilitate the coordination of hearing testimonies or depositions involving OMI Forensic Pathologists and other staff members. By submitting this form, you enable OMI to review your request and initiate the necessary process.


IMPORTANT NOTE REGARDING TRIAL PREP AND PTI's If your request requires pre-meetings related to this activity, such as trial preparation or a pre-trial interview (PTI), please click here to view the list of administrative support staff and contact the appropriate administrative assistant to schedule the meeting. Due to the high demand for testimony and other commitments that our forensic pathologists face, we require at least three weeks' notice. If your request is not submitted within this timeframe, we may not be able to schedule a meeting with the forensic pathologist before trial starts.
