Engagement Req: Project Type*

Please choose the type of project you'd like support with.

Engagement Req: Is this a Just Do It (JDI)?*

A "Just Do It" (aka JDI) is a QUICK WIN where you experiment with your solution and implement the change if it works. This is a quicker, often less complex solution than launching a full project.

If all 4 below statements are TRUE, we recommend you proceed with a JUST DO IT.

1. The Requestor or Sponsor has control over changing or updating the process in question.

2. If the improvement might require a financial investment, the Requestor has available budget.

3. A Sponsor has been identified who has the ability to make both financial and process-related decisions.

4. The impact of the change will primarily be realized by the Requesting Unit/Dept/School.

What does success look like for you? To the best of your ability, please describe the support needed.

Please name your project.

What is the problem you are trying to solve or the opportunity that needs to be realized?

Please enter the name of the person who will Sponsor this project.

What are the quantitative benefits or anticipated ROI of doing the project? Select the following where appropriate:

Not Sure? Review OBT's Lean Metrics Manual to understand different types of metrics to quantify benefits.

Do you already have ideas about how to solve this issue? What solutions have you already tried that haven't solved the problem?

Have any of the anticipated project team members had any experience with prior process improvement projects? Or, have they received Lean Six Sigma training (ex, White Belt, Yellow Belt, Green Belt) at this or any other institution?
