2018-2019 Opportunity Grant Application

Please be sure to complete all fields of the cover page and application form, and include the required cover letter appendices. Opportunity Grant applications for the 2018-2019 year will be reviewed in three rounds throughout the year. Applications for each round must be submitted via the web form no later than November 2, 2018, at 8 p.m. ET; March 1, 2019 at 8 p.m. ET; and April 12, 2019, at 8 p.m. ET. Organizations that have received more than $20,000 from JLW in either the current fiscal year (June 1, 2018 – May 31, 2019) or the previous fiscal year (June 1, 2017 – May 31, 2018) are not eligible for Opportunity Grants at this time. Please note all funds must be spent within 90 days of the award date, and the organization must submit a final report explaining how the funds were used along with proof of purchase. Final reports must be submitted to JLW within 120 days of award date via email to grantsandvolunteers@jlw.org.

C. List of board of directors, including their affiliations. D. Current annual budget. E. Audited financial statement and the Management Letter. If audited financial statements are not available, please state why. F. Most recently filed Form990, if applicable.
