Bridge Week Registration Form

ELIGIBILITY REQUIREMENTS for submitting this form:

Must be an "Active" student to register for Bridge Week, i.e., have purchased your course/program.

You are eligible to submit this form based on how many clinical courses you are taking. Clinical courses are Wound, Ostomy, and Continence (not Professional Practice). Be sure to include Professional Practice in your list of courses, however, if it is part of your program.

ONE clinical course - immediately upon start (purchase) of program

TWO clinical courses - after passing the 1st course exam

THREE clinical courses - after completing one full course

If you are taking more than one clinical course and "fast tracking", i.e., completing ALL clinical courses in 90 day or less, you are eligible to register for Bridge Week upon start (purchase) of program. Choose the "Fast Tracking" option from the eligibility option drop-down menu. If you do not take your first course exam within 15 days, you may be removed from the Bridge Week roster. You will be notified in writing via email if this happens.

If you have questions about eligibility to register, please email Jane Jaudon at jjaudon@emory.edu.

What name do you want on your name tag during Bridge Week?

If you are requesting a Bridge Week date other than your Tentative Bridge Week date:

EARLIER: You must email Jane Jaudon at jjaudon@emory.edu a completed "Planning Ahead" worksheet outlining your plan to finish everything ahead of schedule.

LATER: You will be required to purchase an extension(s) if you do not complete your program in full, including clinical, on or before your Extended Graduation Date.

IMPORTANT: Students must complete ALL clinical courses AND pass ALL comprehensive Final Exams BEFORE attending Bridge Week. NO exceptions. If you are not on track to do so, you will be notified of your removal from the Bridge Week Roster. Please allow sufficient time to take your Final Exams AND retake them if you do not pass the 1st attempt. There is a comprehensive Final Exam for each clinical specialty (Wound, Ostomy, and Continence).

Your second choice must be different than your first choice.
