Group Registration Request Form

This group registration request is for 15+ registrants.

If you have a smaller group please register each individual separately.

This request will generate one invoice to the listed Organization for your group registration.

For separate billing, please submit separate requests.

The American Society for Healthcare Engineering of the American Hospital Association (ASHE), a professional membership group of the American Hospital Association (AHA) agrees to provide group registration to the requesting Organization for the Safe Day One® e-Learning course according to the following:

A code for individuals to use when registering for the e-Learning course.

PLEASE NOTE: Each person needs to register themselves for the e-Learning course. Individual accounts with unique e-mails are required so everyone can create their own online profiles and access their course online in ASHE's Learning Center.

• The code will be limited to the number of requested users below.
• The code can only be used 1 time per user.
• The code is space and character sensitive.
• Codes can be tracked and reported upon request of the contact.

Invoice the organization for the total amount due.

Registration and Login instructions for the organization contact to share with users.

Access and support for ASHE's Learning Center.

Share the registration code with all of the users so they can register themselves for the course.

If someone from your organization is going to register another person, please be sure this is done using the learner's name, email, etc. Accounts must be unique to the learner to ensure access to ASHE's Learning Center.

Pay the invoice for the cost of registration for all of the users.

• The registration fee for the Safe Day One® e-Learning Course is $135 per person (member or nonmember)
• The course fee does not include the exam fee, nor does it register you for the Certified Health Care Physical Environment Worker Exam.
• Participation in the review program does not guarantee a passing score on the exam.
