Section 2: Grant Implementation

Please report the following information as it relates to your use of the Officer Wellness and Mental Health Grant Program funds.

This value should be the total amount spent across all years of the grant.

If no funds were expended in the reporting year, select the category or categories that you will or did spend funds in a different reporting year.


Use the following definitions to respond to these questions:


Physical Health: this includes any investment to improve or maintain officers' physical well-being. Examples include nutrition or weight loss programs, smoking cessation, and physical fitness.

Emotional and/or Mental Health: this includes any investment to improve or maintain officers' emotional and mental well-being. Examples include resilience/wellness coping skills, mental health counseling, suicide prevention, and substance use treatment.

Other: this includes any investment that may improve an officers' well-being that does not fit in either of the other categories. Examples include financial workshops, family support

If no funds were expended during the reporting year, please describe:

1) what you are doing to work towards implementing (Year 1), or

2) what you did in a previous reporting year (Year 2) and if/how they are benefitting officer wellness in the second year .

Include the total number of officers who participated in an activity or program, had access to equipment, or directly benefited another way from the Officer Wellness and Mental Health grant funds.


Section 3: Pre-Officer Wellness Grant Information

In this section, please answer the following questions related to any officer wellness programs that were implemented prior to receiving the Officer Wellness and Mental Health Grant funds.

Did your agency have an officer wellness program established prior to this funding?*

For example, increased positive community engagement, reduction in: work related injuries, sick days used, citizen complaints, days needed to recover from work related injuries, etc.).

Did your agency expand an existing wellness program with this funding*
