The preferred method for collecting Volunteer NDAs is to use the online form, as outlined on the Advancement Hub. In the event that you need to collect NDAs using a paper form, please use the short form below to request access to the most up to date version of the NDA.

You are responsible for creating a spreadsheet with the Names and Advance IDs of the volunteers you have collecting paper forms for, and for sending that spreadsheet to the Advancement Help Desk at advancementonline@virginia.edu.
