1098-T Review request
Complete this online form ONLY if you have one of the following reasons:
- Believe amounts shown in tax form boxes are incorrect
- Didn't receive your tax form
- Tax form is not available in Workday and/or Heartland ECSI
- Request review of information shown on tax form
All requests will be reviewed in the order received.
Requests are reviewed within 7 business days and responses sent to email address on file for the student.
If, for any reason, a 1098-T needs corrected , a corrected 1098-T will be produced and available for pickup or mailed to the student.
Note: The college can not give out tax advice. The college issues the tax form based on transactional data for the tax year per IRS code.

Students can access their yearly 1098T in several ways:
- In workday in the Finances app
- Log into Heartland ECSI (same as RefundSelect access)
- Request tax form mailed to address on record
- Pickup in person
