UpskillOK.org Program Notification Form

Use this form to add, modify, or remove individual programs listed on UpskillOK.org. For:


  • Micro-credentials: Once an institution is authorized to offer micro-credentials, this form can be used for new program submissions or to modify/remove an existing program. For new programs, you might find it helpful to first download a Google Docs copy of the new micro-credential program form to view the required fields or collaborate with others on your campus.


  • Certificates and Degrees: Institutions may only use this form to request listing of an existing approved certificate or degree program via https://UpskillOK.org.


For new micro-credentials and approved certificates and degrees, institutions can choose to list one summative program offering or request individual micro-credentials that recognize specific program components or courses.


Multiple digital badges can also be developed for any single micro-credential, certificate, or degree.


Please do not submit a new offering until you have added it to your institution's landing page.

Program Information

Use the fields below to provide information about your micro-credential, certificate, or degree offering. Keep in mind this information will be used to generate your program listing on https://UpskillOK.org.

Briefly summarize the changes you are requesting for your program.

If not already determined, consult the NCES CIP User Site to find the appropriate CIP code for your program.

For new programs, when is the first offering date? For modifications when is the effective date for the change?

Provide a description (500 characters maximum) basic earning criteria of the program.

Indicate below if your program consists of credit-bearing courses, non-credit coursework, or is available in both offering types.

  • For micro-credentials, list all courses (prefix, number, title, credit hours) or summative activities associated with the program.
  • For approved certificates and degrees, provide a link to the degree sheet.

YOU MUST HAVE A LANDING PAGE FOR YOUR MICRO-CREDENTIAL PROGRAM WHICH MENTIONS ITS SPECIFIC NAME AND PROVIDES INSTRUCTIONS FOR HOW TO ENROLL. IF THIS IS NOT PROVIDED AT THE TIME OF SUBMISSION, YOUR PROGRAM WILL NOT BE POSTED UNTIL THE URL IS AVAILABLE.

Digital Badging Information

All micro-credentials hosted on UpskillOK must be awarded in the form of a digital badge. Institutions may draft their digital badges within the State Regents Credly portal at no cost to them or their learners (access required). Alternatively, institutions may self-host their own badges with an independent platform and provide us with a public link.


Institutions using the OSRHE Credly profile are now asked to draft the digital badge templates in advance of submitting a new micro-credential program. To request access and training resources for drafting digital badges on the OSRHE Credly site, please Request an Account.
