DUHS Student Onboarding

Welcome to Duke University Health System!


A current affiliation agreement between Duke Health and the school program is required. Students are not permitted to begin their experience without required documentation.


Please refer to the email that you received containing this link for the DUHS compliance requirements.

If you cannot locate the email that contains the requirements, please email clinicaleducation@dm.duke.edu to request the instructions and forms/links to complete the student onboarding requirements. The student credentialing process is streamlined for DUHS Staff; please request separate instructions.


Student ID: All students must wear a school-issued photo identification badge while participating in activities on the DUHS campus. School ID badges distinguish students from staff and caregivers.


Student Computer Access: The DUHS preceptor/sponsor will arrange DUHS computer access. Students will be contacted by email to create a Duke computer guest account.


Student Parking: Parking information for Duke University Hospital—including shuttle routes/schedules—is available via parking.duke.edu or by visiting the parking office in Duke South Clinic on Trent Drive. Parking at Duke Regional & Duke Raleigh Hospitals, most of the Duke Clinics, Duke Hospice, and the Duke Fitness Centers is free.

Students are prohibited from taking any photographs while on-site at DUHS.






Annual updates for Core Orientation Training, DUHS Confidentiality statement, TB Attestation, & flu shot are required.

.Duke Employees follow the streamlined process outlined in the onboarding instructions.


•    DUHS Confidentiality Agreement: review in full, sign, note completion, and file at school

•    DUHS TB Attestation Form: review in full, sign, note completion, file at school

•    COVID-19: No longer required

•    Influenza – not required until the 2025-2026 flu season

•    Passport: verify completion of all requirements listed here: Fillable Passport File at school

•    Review and attest to the understanding regarding "Effective Communication Access Accommodations."

•    AHA CPR/BLS: current certification required for students having patient contact file at school

•    Professional Liability Insurance: should be provided by the school

•    Background Check & Drug Screen: access to flagged records must be provided - for DUHS review(refer to the Fillable Passport above). Attest to the requirements

•    Immunization: MMR, varicella, pertussis, and TB test(s) are required (please refer to the Fillable Passport above. All immunizations on file with completed passport. Attest by school

By providing this information, along with the rotation's description/objectives below, you are no longer required to submit a separate Student Participation Form. Providing this information is a requirement of the affiliation agreement.

Provide a brief description of the purpose of the rotation, referencing clinical skills that will be practiced under supervision during the rotation.

Thank you for attesting that the student has reviewed/signed the DUHS forms and completed the compliance requirements. The affiliation agreement defines that these forms be filed and kept by the school, and be available upon request. It is optional to upload them here.
