Summer Food Service Program Eligibility Assessment
The Summer Food Service Program (SFSP) Eligibility Assessment will be used to determine if your organization is eligible to apply for SFSP participation. Eligible organizations must be a public or a non-profit organization, and operators or the organization itself cannot currently be on the USDA National Disqualified List. If the organization or its operators are currently on the National Disqualified List, the organization is not eligible to apply.
Applications for new SFSP sponsor open in January and eligibility assessments are due by March 1st to be considered for the upcoming summer. If this assessment is completed outside of the application time frame, and your organization is eligible, you will be contacted once applications open.
In the event your organization needs to operate SFSP outside of the typical summer months due to an emergency, contact Oregon Department of Education Child Nutrition Programs (ODE CNP) for more information at ode.communitynutrition@ode.oregon.gov.
Assessment Instructions: Please complete the following eligibility questions. You will be contacted by e-mail by ODE CNP about your eligibility status and, if eligible, the application process.
