SCA General Release Form and Assumption of Risk

This release is intended to be used for anyone participating in an SCA film production as cast or crew, officially tying the participant to this project as a requirement of the university. Please read the agreement carefully and type your name at the bottom.

Every student needs to complete the release annually to participate (expiring at the end of Summer Session of the academic year). All non-students are required to submit a newly signed release form per project.

Release and Assumption of Risk

I am participating as on-camera talent or other crew member in the film project being completed by students in DePaul University's School of Cinematic Arts (hereinafter the “Film Project”).

This General Release Form & Assumption of Risk covers the entirety of my participation in the Project.

I acknowledge and appreciate that participating in the Project may require a moderate level of physical activity and interaction with others, and that certain risks are inherent in participating in these activities. These risks include, but are not limited to, personal injury, illness or death, exposure to or contraction of a communicable and/or infectious disease, including COVID-19 (as defined by the World Health Organization and any strains, variants, or mutations thereof), property damages, and property loss or theft. The risks may arise out of accidents or other circumstances, including, but again not limited to, the negligent acts or omissions of myself or others (including DePaul University and its students or agents).

I grant to the student(s) responsible for the Project a perpetual, non-exclusive license to use, modify, reproduce, publish, distribute, and/or publicly display my name, image, likeness, comments and/or voice for any and all purposes related to the Project. I acknowledge and agree that the student(s) responsible for the Project shall own all right, title and interest in and to the Project and all benefits derived from the Project, and hereby waive any opportunity and/or right to inspect or approve the Project, including any use of my name, image, likeness, comments and/or voice therein. I am providing the student(s) with the above rights in perpetuity, worldwide and royalty-free. I do not expect compensation for my participation in the Project. I warrant that my comments are original or that I am otherwise authorized to grant the rights transferred herein.

I HEREBY RELEASE, WAIVE, DISCHARGE AND HOLD HARMLESS DEPAUL, ITS AFFILIATES, PREDECESSORS, SUCCESSORS, TRUSTEES, OFFICERS, DIRECTORS, FACULTY, EMPLOYEES, AGENTS, AND REPRESENTATIVES, PAST OR PRESENT (THE “RELEASED PARTIES”) FROM ANY AND ALL CLAIMS, SUITS, LIABILITY, JUDGMENTS, COSTS AND EXPENSES (“CLAIMS”) ARISING OUT OF MY BREACH OF THIS AGREEMENT, THE INTELLECTUAL PROPERTY RIGHTS I HAVE GRANTED, AND FOR ANY PERSONAL INJURY OR ILLNESS, DEATH, PROPERTY DAMAGE, LOSS AND/OR THEFT, ARISING OUT OF ANY OCCURRENCE DURING THE PROJECT. I ALSO AGREE TO DEFEND, INDEMNIFY AND HOLD HARMLESS THE RELEASED PARTIES FROM AND AGAINST ANY CLAIMS ARISING FROM OR RELATED TO MY OWN ACTS OR OMISSIONS DURING THE PROJECT.
