Athlete Agreement

The South West Sydney Academy of Sport provide high performance training, sport education and personal development opportunities for talented young athletes through a number of sport specific and athlete support programs.


These programs are provided in partnership with State Sporting Organisations (SSO) and aim to assist athletes develop towards the next level of achievement.


By accepting a position within the Academy, and by completing this Athlete Agreement, you are now recognised as an athlete within your sport's High Performance Pathway.


Accordingly, to maximise your experience and potential, there are a number of expectations and responsibility that fall upon you, the Athlete that you are now accountable for.


However, do not worry. The Academy exists to assist you in your journey and will guide you through these expectations and responsibilities.

Please note that a key responsibility of athletes within the Academy is that they maintain and check their own personal email to respond to program sessions and events.


This is also the email the Academy will use to set up the athletes access to the XPS Athlete Management System which includes all program communication including calendar and coach-athlete-parent communication.




Please DO NOT use "School" (ie @education.nsw.gov.au) email accounts if possible, especially if you are changing schools over summer, or do not have access to this email outside of school hours (ie Evenings, Weekends or during School Holidays)

There is no additional cost associated to join the Indigenous Athlete Program. This program aims to provide increased and specific cultural and education opportunities, as well as sport development pathways for Indigenous, First Nation, Aboriginal and Torres Strait Islander athletes within the South West Sydney Academy of Sport catchment area.

Please provide details of condition, medications taken and any details that SWSAS may need to be aware of; or any allergic reactions to certain medications / medicine, and any details of a TUE.


Medications bought to any Academy training session or camp must have clearly marked; Full name, dosage and dosage frequency; and may be required to be handed to the Squad Manager and administered accordingly.


Please ensure you advise your Doctor or GP of your athlete's participation within the Academy, and/or check that the medication does not require a Therapeutic Use Exemption through Sport Integrity Australia.

The Academy's policy on collection use and storage of personal information complies with Australia's Ten National Privacy Principles as defined in the Privacy Amendment (Private Sector) Act 2000. Details of our full privacy policy explaining how the Academy complies with the National Privacy Principles can be obtained from the Academy website https://www.swsas.org.au/policies.html


In summary:


For the Academy to fulfill its obligations to Stakeholders, we need to be given permission to release your child's name, address and scholarship sport to the relevant regional and state sporting association and to the relevant local government council.

The South West Sydney Academy of Sport collects various information to enable processing and assessment of your child's athlete agreements. Some of the information collected will be provided to State Sports Organisations, media outlets, Local Councils and other Stakeholders. In particular, your child's first and last name, suburb where they live and what Academy sport they are involved with. Images and information may also be included in newsletters, web site and other promotional material for the Academy.


You are not obliged to provide the information but if the information is not provided the Academy will be unable to process your application. Any information provided by you to the Academy can be accessed by you during standard office hours and updated by writing to the Academy or calling on (02) 4627 7622. All information provided on this form and gathered throughout the assessment process will be stored on a database that will only be accessed by authorised Academy personnel and is subject to privacy restrictions.

Anti-Doping - Banned Substances Declaration*


Sample collection (also known as doping control or drug testing) is an essential part of protecting the integrity of sport through anti-doping measures. It is the process used to detect the use of a banned substance or prohibited method by an athlete.


Sample collection is conducted by an Anti-Doping Organisation like Sport Integrity Australia, and sample analysis conducted by a laboratory that has been accredited by the World Anti-Doping Agency. In Australia, the analysis is conducted in Sydney by the Australian Sports Drug Testing Laboratory (ASDTL).


Sport Integrity Australia’s sample collection program complies with the World Anti-Doping Code, the International Standard for Testing and Investigations, the Sport Integrity Australia Act 2020 (Cth) and the Sport Integrity Australia Regulations 2020, including the National Anti-Doping scheme.


If you are on either the Sport Integrity Australia or an International Federation Registered Testing Pool (RTP), or another Sport Integrity Australia testing pool, you are subject to testing. In fact, under clause 1.06 of the National Anti-Doping scheme, any athlete who competes in a sport with an anti-doping policy can be tested.


CONSENT TO COLLECTION OR ACCEPTANCE OF SAMPLE UNDER THE AGE OF 18 YEARS


IMPORTANT NOTICE WHICH MUST BE READ BEFORE COMPLETING THIS CONSENT FORM


If you are selected for testing and you are under 18 years of age, you will be required to have a third-party or representative (for example, your parent, your coach or whoever you are comfortable with) to be with you during notification and the sample collection session.


If you do not have anyone to perform this role, another Chaperone or the Doping Control Officer will take on this role and observe the process. In the case of an athlete with an intellectual impairment, it is a requirement that a third party be notified at the same time as the athlete.



By answering Yes, you question you confirm that;

  • I have read the Important Notice Printed Above
  • I understand my responsibilities related to the Sport Integrity Act 2020
  • I understand my responsibilities in complying with my Sport's Anti-Doping Policy
  • I understand my responsibilities as an identified athlete within the Regional Academy of Sport (NSW) High Performance Pathway that I may be identified as an athlete within the Sport Integrity Australia Registered Testing Pool.

Parent/Guardian Code of Behaviour

As a parent/guardian of an athlete involved in any squad activity or session held by the South West Sydney Academy of Sport, I agree to meet the following requirements in regard to my conduct during any such activity, or event:*

To respect the rights, dignity and worth of others.


To remember that Academy athletes participate in sport for their own enjoyment, not my own.


To focus on my child's efforts and performance rather than winning or losing.


To never ridicule or yell at my child, or other children for making a mistake or losing a competition.


To show appreciation for good performance and skilful play by all competitors (including opposition).


To demonstrate a high degree of individual responsibility especially when dealing with, or in the vicinity of persons less than 18 years of age, as my words and actions are an example.


To respect officials' decisions and teach my child to do likewise.


To not physically or verbally abuse or harass anyone associated with sport (player, coach, manager, trainer, umpire and so on).


To in the first instance, direct all of enquires to the manager of your sports program. This will allow the coaching staff to concentrate on their area of expertise.


To understand that as a Parent, Guardian, or family members I shall not become involved in aspects of sport program delivery for example, nomination and selection process, training, and coaching, unless in an officially appointed capacity by the Academy. 


To understand that all team selection enquires shall in the first instance be directed to the manager of your sports program.


To respect the rights, dignity and worth of every young person regardless of their gender, ability, cultural background, or religion.


To understand the repercussions if I, or my child, breach, or are aware of any breaches of, the relevant code of behaviour, member protection, child safeguarding, or other Academy policies.


To ensure that my child attends all compulsory events, including the Academy Graduation and Sports Awards Dinner, and the Council Mayoral Civic Reception in my local government area.


To report any issues that may be related to child protection and safeguarding to the Academy or the relevant authorities as per the respective member protection and child safeguarding policies of the Academy.



Any breach of the above Code of Behaviour may result in the person being excluded from any Academy event, the removal of the Athlete from any Academy event and/or the cancellation of the Athlete's Academy Scholarship.


Athlete Agreement and Conditions of Acceptance

I understand that in order to receive and retain a scholarship I must comply with the following conditions and other directions given by the South West Sydney Academy of Sport:*

1. I will comply with and obey all lawful instructions given to me by Academy personnel, or any other person in authority with the Academy.


2. I will ensure that all appointments, training sessions, competitions, and other meetings arranged for me are attended and attended punctually.


3. I understand that the Academy Graduation & Sports Awards Dinner and The Civic Reception in my local government area are compulsory events.


4. I undertake to advise the Academy as soon as possible of any obvious danger within the Academy of which I am aware, or of any activities conducted by any person who may endanger the property or persons of the Academy, or those associated with it.


5. I understand that my position within the Academy will be in jeopardy if I engage in any illegal activities, particularly the use of performance enhancing drugs and doping practices.


6. I will at all times act in an appropriate and sporting manner and maintain a proper code of behaviour, particularly during Academy activities and whilst representing the Academy. I understand that any breach of conduct, or misbehaviour may result in the termination of my scholarship.


7. I will keep the Academy informed of any medical conditions and treatments that could affect my performance and of any drugs or prescriptions that may have been prescribed for me.



8. I acknowledge that at all times; I must comply with the Academy's directions and requirements and in the event of any breach of conduct or misbehaviour on my part, and my scholarship may be terminated immediately.


9. I understand that the Academy may terminate my scholarship, or vary the benefits associated with it, by notice in writing.


10. I acknowledge that it is compulsory for me to wear the appropriate supplied Academy clothing at all events and not wear any clothing which may identify any sponsors in conflict with the official Academy sponsors. I also agree that my Academy items will not be worn or used by anyone but myself and will be maintained in good order.


11. I agree to return all clothing provided to me by the Academy in the event of my not completing the program.


12. I will assist in the promotion of the Academy and it's official sponsors.


13. I acknowledge that I must travel and/or stay with the Academy squad whenever transport and accommodation are provided by the Academy as part of the program content.


14. I will notify Academy staff of my travel arrangements to and from Academy events where appropriate


15. I am aware and agree to the Academy's policies surrounding Media and Privacy and agree to allow my image to be used in the promotional activities of the Academy as set out in these policies.


16. I recognise that a range of other Academy Corporate partners are funding my athlete levy and will respect their support through my good behaviour and attendance at training and on game days.


17. I understand that the South West Sydney Academy of Sport will not meet the cost of any medical, dental, or health service required, and it is my responsibility to ensure that I have adequate Private Health Insurance and appropriate extra cover at all times.





Any breach of the above Code of Behaviour may result in the Athlete being excluded from any Academy event, and/or the cancellation of the Athlete's Academy Scholarship.



This is similar to a passport photo, or selfie. This image will be used for the Athlete ID Card.
