2025 LGBTQ2SIA+ Student Success Plan Revision Survey

The Oregon Department of Education is planning to revise the LGBTQ2SIA+ Student Success Plan from 2020 in order to be more aligned with other Student Success Plans and to better reflect the current needs of LGBTQ2SIA+ students and youth in Oregon for the next five years 2025-2030. To assess the current needs of Oregon youth, school districts, and community partners, ODE is releasing this LGBTQ2SIA+ Student Success Plan Revision Survey before revising and developing the new plan.


All survey questions are optional and anonymous. To protect the safety of school and community members and youth, please avoid sharing any personally identifiable information in this survey. If you disclose any information about abuse and provide any personally identifiable information, ODE is obligated to report the abuse.


If you or a young person is experiencing abuse or harm, you can seek immediate assistance by reporting to a trusted teacher, counselor or adult, or calling 911. To report suspected child abuse, call the Oregon Child Abuse Hotline at 1-855-503-SAFE (7233). If a student is experiencing discrimination at school based on gender identity or sexual orientation (or any other protected class), you can report it to your local school district or reach out to ODE for assistance by calling 503-551-5713. Additionally, People of all ages who experience hate, bias crimes, or discrimination can report them using the Non-Emergency Bias Response Hotline at 1-844-924-BIAS (1-844-924-2427)
