AIHA Laboratory Accreditation Program (AIHA LAP, LLC) is always looking for suggestions on how it can improve its program. One of the more common suggestions received is that one or more of the AIHA LAP, LLC policies needs to be changed. These suggested changes can be as minimal as correcting a typo, to a change as drastic as eliminating a requirement. AIHA LAP, LLC encourages this feedback and this form has been generated to allow these suggestions to be easily submitted to AIHA LAP, LLC for consideration. At any time, any AIHA LAP, LLC stakeholder can request a change in Policy, appendix to Policy, or other Policy-related documentation via this form. This form is submitted with the understanding that not all suggestions will be enacted by AIHA LAP, LLC.
