ENROLLMENT VERIFICATION LETTER REQUEST FORM


This form can be used for, but is not limited to, the following:

  • Getting a driver’s license
  • Opening a bank account
  • Visa extension/renewal

How to request an enrollment verification letter

1) Make sure you are:

  • registered for a minimum of 12 units and fully paid/on a payment plan for the current/upcoming semester; or
  • approved for a reduced course load and fully paid/on a payment plan for the current/upcoming semester


2) Make sure to update your U.S. address in your WebSmart before submitting this form.


  • Go to My Profile.
  • Click "View/Update Personal Information." (You will get taken to a new page in a new tab).
  • Click "Personal Information."
  • Scroll down to the Address/Phone Number section.
  • Click "Add New".
  • Select "Mailing" for updating your U.S. address.
  • Enter your information by filling in the right fields.
  • Click "Add."


3) Submit this form online.


4) ISP will send you the enrollment verification letter via e-mail within 2-3 business days.
