This form is intended for Faculty at UNM's Main and Branch campuses, as well as University-wide Graduate Student Assistantship Holders, to inquire about leave offered under the Family Medical Leave Act (FMLA) or upload applicable medical certification documents associate with FMLA.

Additionally, Faculty Administrators are required to use this form to submit leave requests on behalf of faculty after obtaining all required signatures on the corresponding leave forms.
