Start of Semester (S.O.S.) Form

INSTRUCTIONS: Please upload a copy of the syllabus for each course that you are teaching and ensuring the following:

  1. Naming Convention: Name your syllabus file(s) using the course ID number and the CRN. Example: “MUS10B-43770”
  2. Be sure your syllabus includes your: -Mt.SAC contact information -Office hours and location (Required: Fall & Spring) -SLOs for your course -Statement of accommodations for students with disabilities
  3. Deadline: Upload all of your syllabi below no later than the end of the FIRST week of the semester.


IMPORTANT NOTE: Accreditation requires syllabi to list course Student Learning Outcomes (SLOs). Therefore, please add your SLOs. The words "Student Learning Outcomes" must be listed on your syllabus.


**Per Article 18.B of the Faculty Agreement, submitting records in an accurate and timely manner is a contractual faculty obligation. Failure to submit accurate records by the established deadlines may be reflected on your faculty evaluation.**

Your final exam should be administered during the 16th week of the semester, or last day of class for short-term classes.


*Final exams must be given for both lecture and lab portions for classes that have been granted parity. If you have questions about whether the course you are teaching has parity, please contact your Department Chairperson.

10.A.1. Full-time professors must be on campus or at other assigned locations for a minimum of two (2) hours per day for a minimum of three (3) days, but not more than five (5) days, for each week of the adopted calendar. On campus, assignments include teaching, office hours, and other duties as mutually agreed upon between the professor and their division dean or designated educational administrator. This assignment may be met on Saturdays or Sundays by mutual agreement between the professor and the division dean or designated educational administrator. Through a mutual agreement with the division dean or area educational administrator, the third day on campus obligation may be met through remote synchronous office hours.

10.Q.6. Four-day Work Week: Professors who have more than five (5) weekly hours of overload and/or extra pay assignments shall be required to work a 4-day work week on campus or at other assigned locations and may be met by a minimum of two (2) hours per day for a minimum of four (4) days, for each week of the adopted calendar. Through a mutual agreement with the division dean or educational administrator, professors may meet one (1) of the four (4) required on-campus days synchronously through technology communication.
