FERPA Verification Form

Use this form to submit your documentation for training and acknowledgement of FERPA compliance.

Instructions

Please submit the following for your FERPA training verification:

Certificate of Completion for FERPA Fundamentals for Merging Canvas Classes in POD Connect.
Completed checklist from this form detailing your Canvas course, navigation, assignment, and additional settings.

Upon review of your submission, you and your division will be notified that you have completed the requirements. Please allow up to 48 hours for verification review.

NOTE: If you have already submitted this form, please do not submit again.

Settings for multiple sections of the same class in the same modality

Canvas Course Settings*

In your Course Settings, click More Options. Mark ALL of the following once complete:

"Let students create discussion topics" is not checked.

"Let students organize their own groups" is not checked.

"Disable comments on announcements" is checked.

"Hide grade distribution graphs from students" is checked.

Canvas Course Navigation*

In your Course Settings, go to the Navigation tab. Hide the following, save your settings, and mark ALL of the following once complete:

People

Chat

Conferences/Big Blue Button

Canvas Course Assignments*

These assignment settings will ensure FERPA compliance. Mark ANY of the following that apply to your course:

Assignments are assigned to each course section.

Discussions are duplicated and assigned to each course section.

Discussions are created by groups based on section CRNs.

Peer Reviews have students manually assigned from the same CRN.

Group assignments are created based on section CRNs.

Canvas Collaboration groups are manually created based on section CRNs.

Additional Canvas Settings*

These additional settings will ensure FERPA compliance. Mark ANY of the following that apply to your course:

Comments are disabled for Canvas Studio videos.

Comments are enabled for Canvas Studio videos, but videos are duplicated and posted to each section individually.

Inbox Messages sent to multiple students have the "Send an individual message to each recipient" option selected.

All Group settings have the "Require group members to be in the same section" when creating a group set.

Synchronous Zoom sessions are held separately by section/CRN.

Posted recording of synchronous sessions do not include student data: names, images voice, phone number or chat comments.

Third party tools/external apps have been vetted for FERPA compliance.

I understand that combining two or more sections of the same course into one Canvas course shell is a privilege that I am allowed so long as I take every precaution described below to ensure that student privacy is protected under the requirements of the Family Education Rights and Privacy Act.

I understand that if I fail to exercise care in applying these restrictions to protect student privacy and adhere to the requirements of FERPA, my privilege of combining sections into a single course shell may be removed.

By submitting my documentation on this form, I acknowledge that I will adhere to federal FERPA requirements to safeguard student privacy for merged course sections.
