Thank you for your interest in providing public comment at an upcoming Developmental Pathways' Community Advisory Committee (CAC) meeting.

Please complete this form to help us prepare for your participation. While this form is recommended, it is not mandatory; you can still provide public comments without prior notice.

Note: Before filling out the form and attending the meeting, please be sure you have addressed your concern with the Case Management team through the proper channel. For more information on how to connect and share a problem, please visit our feedback webpage.

To help prepare for your comment, please review the following recommendations developed by the CAC:

• The CAC will dedicate 15-30 minutes for public comments at the beginning of each public CAC meeting.
• Public comments are limited to three minutes per person, with accommodations as needed and approved by the CAC.
• Time for public comments will be at the beginning of each meeting. If the allocated time is not fully utilized, the meeting will proceed without delay.
• We request pre-registration for public comments 24 hours before the meeting. “Walk-ins” (in-person or online) are welcome if time and capacity allow.
• Expectations and guidelines will be provided with anyone attending a public meeting who wants to speak.
