2025 ENPDC Contact Hour Request Form

Emory Nursing Professional Development Center (ENPDC) is accredited as a provider of continuing nursing professional development by the American Nurses Credentialing Center's Commission on Accreditation. To request contact hours for your activity, please complete the following request form.

Note: Any requests external to Emory Healthcare or Emory University Nell Hodgson Woodruff School of Nursing will incur a fee to process and review the application. The fee is based on the number of contact hours requested for the activity:

0.5 - 4.9 hours: $100
5.0 - 9.9 hours: $150
10 - 19.9 hours: $200
20 - 29.9 hours: $250
30 - 39.9 hours: $350
40 - 49.9 hours: $450
50 - 59.9 hours: $600
60+ hours: $750

2025 Blank Documents can be accessed in the Teams Portal. If you do not have access to the portal, please complete the official ENPDC Nurse Planner training that can be found in the ENE Course Catalog.

Activity File Template
Financial Relationship Disclosure Forms (if activity is clinical)
Activity Flyer or Marketing Email/Material that includes ENPDC Disclosure Statements
Agenda (if activity is over 3 hours long)
Sample Certificate of Attendance
