CCSD and NYE County COI's can be found at girlscoutsnv.org under forms, and can be printed without filling in this request form. CCSD and NYE County SD COI's are automatically renewed October 1st each year.

Copies are posted annually to girlscoutsnv.org as soon as they are provided by our Insurance provider. Fill in this form when a location other than CCSD or NYE County SD requests a COI (Certificate of Liability Insurance) for a meeting or event location.

COI requests for locations other than CCSD and NYE County SD must be requested using this form annually at the start of the Girl Scout year (Oct 1st).
