Resignation Submission

Please be aware that part of this process will require you to turn in a signed letter of resignation that includes your name, last date to work, employee ID number, the reason for resignation and a handwritten signature if possible.

Employee Document submission

For Employee, please submit your signed letter of resignation. Your letter should include a handwritten signature and your anticipated last day to work.

Supervisor Document Submission

Please upload any documentation that you have related to resignation/termination including attempts to contact employee, screenshot of text resignation, audio file of verbal resignation etc.

Resignation letter should contain: name, last date to work, Employee ID number, reason for resignation and handwritten signature if possible. Supervisor and/or HR please submit documentation.

I acknowledge that, by submitting my intent to resign prior to the end of my 2024-2025 assignment, I no longer qualify for the Board approved new hire bonus previously paid to me upon my hire. I also understand that employees who resign prior to the end of their 2024-2025 assignment after receiving the new hire bonus are required to repay the bonus amount, and that this amount will be deducted from their final paycheck.
