This form is designed for the CE Trainer (formerly called CE Liaison) at the HMIS participating agency to request access for a staff member to become a Housing Assessor. All sections and training must be completed or scheduled.

If the ACT has not been completed, please confirm the date the new assessor will attend. Choose the soonest training the assessor is able to attend. Completion is required within 90 days of approval.

The Monthly Assessor Credential Training is hosted virtually on a monthly basis, and is required once a year for all assessors. The training is designed to help assessors of all levels (new & seasoned) learn how to navigate Coordinated Entry and how to best serve the clients on their CE caseload.
