Chromebook Pickup Form

**NEW- Please use the following Google form to print (DO NOT WRITE ON GOOGLE FORM) and attach to the red bins prior to pickup:


  1. Before completing this form, please ensure that all Chromebook's Status is "Out for Repair."
  2. Fill out the following form if your school has Chromebooks that need to be picked up.
  3. Requests can be declined at the discretion of the DMT members.


NEW INFORMATION PLEASE READ CAREFULLY:


*No more than 2 bins will be picked up on a weekly basis.

*Bins can hold up to 20 Chromebooks, do not exceed this amount.

*All pickups MUST be ready on the site's weekly delivery day to avoid any delays, and must be collected from the sites BSS/POS designated loading area.

**Please note that all bins will be picked up from the BSS or POS loading area ONLY.
