Direct Support Professionals employed by UCP Central PA can use this form to request authorization to enroll in mobile device management and access their email via the Microsoft Outlook app on a mobile device. Authorization and usage is subject to device compatibility and agreement of terms and conditions set forth in the UCP Central PA Employee Handbook section titled USE OF INFORMATION TECHNOLOGY.
