This form is for the Main Contact of a Community Organization with Minor Volunteers (17 yrs and under). The Parents/Guardians must sign release form and group contact can attach to this registration with minor Volunteer Roster. Minor volunteers cannot begin service until all documentation is received by the Office of Volunteers.
