3. Completed Accounts Receivable Clearance Application (Required)

4. Most Current Federal Tax Return (1st page only- If applying for DSB, please provide prior 3 years) (Required)

5. Copy of Current Lease or Deed (Required)

6. If applicable, Business Roster (Detroit Headquartered Business only)

7. If applicable, Minority Based or Women Based Enterprise Declaration

8. If applicable, for Detroit Resident Businesses, please upload your employee roster showing Detroit resident employees, and proof of each individual's payment of the City of Detroit Income Tax in the previous taxable year, or proof that the individual is now subject to payment of Detroit Income Tax due to his/her residence in the City of Detroit.

9. If applicable, Additional Detroit Locations -Upload Additional Locations in a Separate Document Here. Please include total employees and total Detroit resident employees for each document.
