2. Most Current Federal Tax Return, including Schedule K-1 adding up to 100% (If applying for DSB, please provide prior 3 years) (Required)
3. Copies of Current Lease or Deed and Previous Year's Lease or Deed (Required)
4. Copy of Operating Agreement or Articles of Incorporation (Required)
5. Most recent City of Detroit Corporate Tax Return (Required)
6. If applicable, Business Roster (Detroit Based and Detroit Headquartered Businesses only)
7. If applicable, Minority Based or Women Based Enterprise Declaration
8. If applicable, for Detroit Resident Businesses, please upload your employee roster showing Detroit resident employees, and proof of each individual's payment of the City of Detroit Income Tax in the previous taxable year, or proof that the individual is now subject to payment of Detroit Income Tax due to his/her residence in the City of Detroit.
9. If applicable, Additional Detroit Locations -Upload Additional Locations in a Separate Document Here. Please include total employees and total Detroit resident employees for each document.
10. If applying for Detroit Resident Based Business certification, photo IDs of all Detroit resident employees. All required documents must be uploaded for application to be processed. Document templates can be accessed by clicking the respective links below:
