Homeowner Assistance and Reconstruction Program (HARP) Eligibility Questionnaire

Please complete and submit the Eligibility Questionnaire to determine if you are qualified to apply for HARP. This questionnaire should take about 15-30 minutes to complete. Do not close or refresh the page until you are finished and have submitted your responses.


The information you provide will be used to determine your eligibility and whether your household is invited to apply for HARP, so read carefully and fill out the questions below as accurately as possible. If you are not eligible, you may qualify for other ReOregon programs and will be notified when they open.


Do not submit more than one form per household. Multiple form submissions will not speed up the process but slow it down as we verify the submissions. If you need to correct your submission or are unsure if you have already submitted a form, please call or email us.

When counting household members, please only count people who will be living with you if you are eligible for HARP. Household members are defined as adults and children, or individuals who share the same house, whether they are related or unrelated, and regardless of age. Please do not count temporary members in the household, such as foster children, wards, and employees, or tenants.

Each year, the US Department of Housing and Urban Development (HUD) calculates the area median income (AMI) for each county. The household income definition used is annual income as defined by the IRS as “Adjusted Gross Income." (For the full definition click here).


The table below displays the three AMI levels for households of one to eight people in the county you selected.

If you selected "Other" as your county, you will need to use HUD's website to find the income level for your household in order to answer the question below.
