The Self-Classification Questionnaire for Biological Items is used only for the shipping of the following types of items:
- Biological materials (e.g., seeds, algae, lignin, and human and animal tissue)
- Biologically-derived materials (e.g., cell lines, plasmids, and toxins)
- Genetic materials (e.g., DNA and RNA)
This form must be completed by the individual, researcher, or PI (“Sender”) from which the request for shipment originates. By completing this self-classification form, the Sender certifies that they have carefully reviewed the control lists provided below, and answered all questions truthfully.

Berkeley Lab Shipping and EHS Export Review Team will be notified at the same time you receive export control approval for your shipment.

If this export self-classification form is for multiple items in a single shipment, Section A should list only those items which can be classified as either EAR99 or non-EAR99 as determined in Section C. Each non-EAR99 item requires its own form.
If your shipment is to Belarus, Cuba, Iran, North Korea, Russia, Syria, or Ukraine, DO NOT submit this form. Instead, contact us directly at exportcontrol@lbl.gov.
For non-biological items, complete the "Export Compliance Office International Shipping Request” form.
