Beyond the Bell/Club B (BTB/CB) partners provide reasonable accommodations to people with disabilities. If you require an ADA accommodation to participate in a BTB/CB program, please complete and submit this request form. Requests should be made at least two (2) weeks prior to the date the program begins. ADA accommodations will be determined on a case-by-case basis. If you need assistance in completing this form or have questions, you can meet with Parks Tacoma’s customer service staff at any Parks Tacoma Community Center or at Parks Tacoma Main Offices. You can also contact staff at 253-305-1030. Please note, the ADA does not require BTB/CB partners to take any action that would fundamentally alter the nature of its programs or services, or impose an undue financial or administrative burden.


Directions: Please complete as many fields as possible. Assistance completing this form is available from any customer service staff.


One participant per form. Up to three activities can be on the same form if they require the same accommodation.


We are unable to provide 1:1s, staff administration of medications, hands-on personal care in the bathroom, or restraint as a behavior response for participants. Those requiring this kind of support can request a caregiver attend with their participant.


Individuals receiving accommodations must still comply with program behavioral expectations with or without reasonable accommodations and not present a safety concern to themselves or others.


Submission of this form does not guarantee the requested accommodation. Staff will reach out with accommodation decisions as soon as possible.

Please detail what supports your child might need (e.g. sensory tools, key phrases, visuals) in their program settings in addition to the typically available supports in order to be successful.
