Agency Pre-Monitoring Visit Form

Thank you for your partnership with Feeding San Diego! Your agency is due for a monitoring visit. Please complete the following form and include digital copies of the required documents.


Please include the best day(s) and times for your in-person monitoring visit. As a gentle reminder, visits should be scheduled during your distribution day. A surprise visit may be necessary if this form is not filled out within 30 days of the request.

Distribution Management

In this section you will ask about your distribution style, dates and times, and operations.

Pantry- clients shop for themselves during open hours or by appointment only

Large Scale/Farmers Market- food is set up for participants to walk up or drive by

Homebound- food is delivered to homebound participants

Meal-service- meals a prepared for clients

Residential- participants live on-site

Mobile- Food is distributed from a van/truck

Food Rescue

Only complete this section if your agency participates in the retail rescue program. You will be asked to provide your pick up schedule, and your cold storage types.

Compliance

In this section you will be asked about your pest control plan, dry and cold storage types, number of cold storage units, vehicle types, signage, Department of Environmental Health Permits.


REMINDER: All areas storing cold and dry foods should always be kept in the required temperature ranges

Refrigeration: 32°F – 40°F

Freezer: 0°F and below

Monitoring Visit Scheduling
