Proclamation Request Information

This event appearance request form does not serve as a proclamation request, if you would like Mayor Frey to present a proclamation at your event, please fill out the request form found at https://app.smartsheet.com/b/form/99e1b724d5aa4888ae7c0a7d57a71226.

Please note, we require 10 business days to process requests for proclamations. A member of our team will reach out to you to finalize any remaining details and make arrangements for you to receive the prolamation. If you are looking to have this proclamation in fewer than 10 business days, please reach out to mayor.schedule@minneapolismn.gov. While we can not guarantee that we will be able to accomodate your request in less time, we will do our best to meet your needs.
