Charitable Donation Request Form
The Golf Club at Yankee Trace has established a program to support area charitable organizations by offering donations for goods and services provided by the golf course and restaurant. These donations are intended for use as raffle items, silent auction prizes or winnings that participants may receive through events organized by the charitable organizations.
Recognized 501(c)3 charitable organizations requesting a donation must complete the form below and provide the following by uploading the documents at the bottom of the form:
- Provide verification of their 501(c)3 status.
- Submit a brochure, website, or other literature that verifies the date, purpose, and location of the event for which the donation is requested.
Organizations can request a donation once per calendar year for one (1) of the following:
- Golf Rounds: Two (2) complimentary 18-hole rounds of golf with a $5 cart fee per player. Valid Monday - Thursday after 12:00 p.m. ($110 value)
- Sunday Brunch: Two (2) complimentary Sunday brunches. Holiday and special brunches are excluded. ($60 value)
- Indoor Golf Simulator: One-hour indoor golf simulator rental. ($45 value)
Application Procedure:
- Complete the donation request form.
- Provide the following documentation to the City of Centerville:
- Form verifying 501(c)3 status.
- A brochure, website, or other literature verifying the date, purpose, and location of the event.
- This should be done in a timely manner, generally 14 days before the date of the event.
Donations are not guaranteed and are subject to City Manager approval. Donations expire one year from the issue date.
Please allow a minimum of two weeks for processing and delivery of certificates.

The City is limited in providing donations by its financial position and accessibility to the golf and restaurant facilities.
