Boise State Office Equipment Checkout

The intent of this form is to track Boise State University property that has been approved to be taken home or to a remote location, on a short or extended basis. Property used for educational or work purposes must be returned to the University prior to the end of employment or agreed academic term. Participants assume the cost of unreturned or potentially damaged property (subject to review). Departments are encouraged to track equipment and inventory throughout the year. Checkout Terms: By checking this agreement I acknowledge that I have received the equipment in good condition and accept full liability for damage or loss. This includes concealed or unreported damage upon return. If the equipment is not returned by the date and time agreed upon, I agree to pay any associated fines and understand that a late return or lack of return may result in a suspension of checkout privileges, academic consequences or legal repercussions. If equipment is lost or damaged and the student is unwilling or refuses to make payment, an encumbrance will be applied against their student fees account to prevent them from registering for future classes until payment is made.
