ID Card Request

Please use this form to request your first copy or a replacement copy of your student ID card. * ID cards requested through this form will be mailed to the local US address listed in MyEnsign for students who will be attending Ensign College in-person. * It may take 1-2 weeks to receive your new ID card. * If you want to pick up a student ID card in person, please come to the Registration Office on the second floor of Ensign College. Replacement cards cost $10 and will require a receipt. Contact the Cashier's office at 801-524-8153 to pay the fee and get a receipt.

I uploaded a photo for my student ID that meets the three requirements listed above.

Upload a recent picture for your student ID: (1) Upload a png or jpg file; (2) Follow the dress and grooming standards of the Ensign College honor code; (3) Mimic the crop and background format of your government issued photo ID. For example, the picture should be from the shoulders up with a solid, light-colored background.
