Chapter CHFM/CHC Exam Fee Voucher Request Form

Please complete this form to request CHFM and/or CHC exam fee vouchers that were awarded as part of the ASHE chapter award program. Before completing this form, please make sure you:


  • Meet the criteria for taking the CHFM or CHC exam as outlined via the AHA Certification Center.
  • Are an active ASHE Member.
  • Are a first-time certification exam taker (i.e. vouchers cannot be used for certification exam renewal).
  • Are a first-time voucher recipient.


Individuals who do not meet the above requirements will not receive a voucher.


Once you complete and submit this form, an ASHE staff member will connect with you within 5-10 business days.


All voucher requests are due by October 31, 2025. Requests received after October 31 will not be fulfilled.

Note: Only Chapters who participated in the 2024 Chapter Awards are listed. If your chapter name is not here, please contact Jenny Hull at jhull@aha.org.
