First Class Dance Academy Waiver and Release of Liability

By signing this form, I acknowledge and assume all risks associated with participating in a trial class at First Class Dance Academy, including risks of injury, illness, accidents, or other unforeseen events. I waive, release, and discharge First Class Dance Academy, its employees, and volunteers from any liability related to injuries, accidents, or other issues arising from participation in the trial class. I certify that my child, ________________________________, is in good health and able to participate. I agree not to hold First Class Dance Academy or its affiliates responsible for any injuries or damages incurred during the event.

I have read and fully understand this waiver and voluntarily agree to its terms. If the participant is a minor, I, as the parent or guardian, consent to the participation and agree to these terms on behalf of my child.
