Supplier FAIR Submission Form

Introduction

Dear Valued Supplier, If you’ve received this SmartSheet link, then Delta Flight Products has requested you submit a First Article Inspection Report (FAIR). Please provide the information requested in the form below. There is a field to attach your completed FAIR and supporting documentation (e.g. raw material certifications, special process certifications, and/or acceptance test reports). The completed FAIR must be signed by an authorized representative of your company. You may submit any format for the First Article Inspection Report that your company uses, but we require that it contain equivalent information as shown in AS9102 Rev C. AS9102 utilizes 3 forms. Form 1 for Part Number Accountability (including sub-components), Form 2 for Materials, Special Processes, and Functional Testing, and Form 3 for Characteristic Accountability, Verification, and Compatibility Evaluation (i.e. 100% dimensional inspection and verification).
