This form is to be completed and submitted by the Graduate Coordinator to indicate that the records for the below-mentioned student have been reviewed and that all of the requirements for graduation have been met. Graduate Coordinators should request an updated final Program of Study from the student to review alongside the student's academic record in MaineStreet. A final Program of Study should be submitted to the Graduate School along with this form for final review.
