This form is to ask the Jefferson County General Administrator to ACCEPT their priority right to serve as Personal Representative in the below-referenced estate. In assessing this request, the Administrator will need additional information from you in order to consider your request. Please complete the form below to assist them in their review.

The Administrator's Office will work to process your request in an expeditious process, but no guarantee is made on the timing of request to open an estate by the Administrator. Please provide the full name of the Decedent exactly as you will use want it for opening the Estate. This will prevent more than one name being used for opening an estate, causing the court, the administrator's office, and you additional time and effort.

I agree to supply the Jefferson County General Administrator any necessary documents, including the following, if they agree to open the decedent's estate based upon my request. Such Documents should include, but are not limited to, the following:


  1. Multiple certified copies (5) of Original death certificate.  (copy emailed upon acceptance)
  2. Original of known wills. (copy emailed upon acceptance)
  3. Obituary of the deceased including obituaries of deceased heirs.
  4. Divorce Decrees of the deceased
  5. Veteran Records of the deceased (DD214)
  6. Birth Certificates of Children (Born and Adopted)
  7. Income Tax Statements
  8. Asset Statements (Bank, Investment, Deeds)
  9. Liability Statements (Debts, Mortgage, Funeral, etc.)
  10. Probate Filing Fees
  11. Newspaper Publication Fees
  12. Excess Bond Fees


You will receive email notification to supply the requested documents below. Please attempt to retrieve as many documents as possible in your first submission to our email request. An electronic version of the death certificate can be submitted at administrator@greenelawfirm.com

I have listed all heirs in this document. I have not omitted any heirs after a thorough and diligent search of heirs. I have listed location unknown for any heir I do not have contact information for. I have listed heir as deceased if they are not longer living and stated who that person's heirs are.
