This form is to ask the Jefferson County General Administrator to ACCEPT their priority right to serve as Personal Representative in the below-referenced estate. In assessing this request, the Administrator will need additional information from you in order to consider your request. Please complete the form below to assist them in their review.

Please only list none or unknown if the field is immaterial to your estate. The administrator will require further information if all material information known about the estate is not supplied in the first request to open an estate, which can lead to additional delays for the estate.

The Administrator's Office will work to process your request in an expeditious process, but no guarantee is made on the timing of request to open an estate by the Administrator. Decisions by the Administrator will automatically generate a response back to your email on the granting or denial of your request to open an estate.
