If you are graduating from one program and will begin a new program or degree level at SEU, or will return to SEU after OPT, you will need to complete this form and upload a new Certificate of Finance form and Banking Certification form or bank letter to receive an updated I-20 for your new academic program. You must submit this form by your graduation date or OPT end date.
