2021 Personal Workload Survey

Thank you for taking the time to fill in our retrospective annual survey of procedures performed. At the end of the survey you would have received some kind of a "completion message" that you were instructed to screenshot (keep a record of). This is where you are to UPLOAD that screenshot to receive your RECEIPT NUMBER. Please transfer this receipt number into your CPD Wizard - PWS record to validate your record for 2021. Your RECEIPT NUMBER will be presented on completion of this form, as well as via email (IF) you tick "send me a copy of my responses" box.
