Labor Commission Wage Claim Intake Form

If you have received a wage claim from the Labor Commissioner's Office regarding lost or incorrect wages for an employee, complete the intake form, and submit a scanned copy of the wage claim document.

Name of employee who submitted the wage claim

Provide Employee ID, if known. This is helpful for identifying the employee, particularly in cases where there are multiple people with the same name.

The wage claim # provided by the Labor Commissioner's Office, if given.
