This form is for ambulance services to request Pediatric Safe Transport Devices. Please note that SD EMSC provides these resources to ambulance services in South Dakota, and requests that your service have a designated Pediatric Champion. If you have questions about this, please reach out to our office.

Each agency can request one device per ambulance that they have in service. If your agency has previously received devices from our office, please note in the additional information box why you need additional devices.
