Food Truck/Catering Request Form

This form to be completed by the individual(s) requesting food truck/catering to come onto the FOX Studio Lot. The link below is the list of the current "Pre-approved" Food Trucks. If a truck is found on this list, it means FOX has all forms from them. Please save each file as: "Food truck/Catering Name_nameoffileuploaded" (Example: FoodTruck_CertificateofInsurance). 1. Certificate of Insurance (if not already on file) 2. Existing Contract (if applicable) 3. Current LA County Health Permit 4. Current LA County Business License 5. Current Food Handlers Certificate Depending on the information provided, follow-up may be required. This form needs to be submitted at least 3 days before the event date. Risk Management

FOX's Risk Management Department requires a copy of the current Certificate of Insurance as well as the existing contract (if applicable). The Certificate of Insurance must match the terms of FOX's standard insurance requirements.
