Students who have utilized the incentive program during the Draws and need to drop their scheduled clerkship(s) in P11 and/or P12 of 2024-25 are required to complete this form for approval from the Committee on Curriculum and Academic Policy (CCAP).

Students must select the period to which they wish to move the clerkship. It is important to note that all 10 core clerkships should remain on the student clerkship schedule at all times. To identify available periods for moving a core clerkship, please check Core Clerkship availability in the MEP Clerkships system (under Clerkships/Shuffle).

After the first day of the clerkship, students should not use this form. They should consult with the Clerkship Director directly to discuss the circumstances and options.
