NOTE that volunteers & resources are limited and we will only be able to provide services to families who are able to adhere to the program guidelines.
Parents/legal guardians must register their child, agree to follow the policies and guidelines, discuss with their child the expected behavior, and read and sign this form.
Students must bring homework to work on with their tutor during the time they are receiving help; Volunteers and Library Staff will not be able to provide activities or materials for these sessions. Students who consistently attend without homework to work on will be asked to leave the program so that others who have homework can participate.
Students must be respectful of their homework helper and library staff. Disruptive behavior will result in the student’s removal from the program.
For In-Person Homework Hub: Please note that minors age 10 and under must be accompanied by a parent or guardian.
I have read and agree to follow the above guidelines for participation. I will ensure that my child knows the rules and follows them and that repeated failure to do so will result in my child being removed from the program.
I give permission for my child to participate in LeRoy Collins Leon County Public Library’s Homework Hub program. I understand that my child will be assigned to a volunteer tutor who has undergone a Level 1 background check. A Level 1 Background Check is a state-only name-based search of the volunteer’s employment history, state and local criminal history and whether or not the volunteer’s name appears on the national sex offender registry.
