As a Medicaid member, you may choose your own PCP. You can change your PCP without cause 2 times per year. You are given 30 days from the date you receive notice of your PCP assignment each year to change your PCP without cause. You are allowed to change your PCP without cause up to 1 time per year thereafter. You are allowed to change your PCP with a cause at any time.

By submitting the PCP Change Request form, staff from the Member and Recipient Service Line will reach out to help you change your PCP. We can help arrange your first PCP visit and transportation to get to and from your appointment.
