Welcome to the Interactive map for Transitions and Continuity of Care (ITACC) project. This platform has the vision of enhancing transitions, continuity, and access to care across the 18 westernmost counties in NC. With the hope that our communities have access to health care services and beyond, this map includes reproductive healthcare programs as well as those that treat substance use disorders. A few other noteworthy indicators are substance use disorder treatment locations, hospitals (L&D availability), and safety net services.


The information, hyperlinks, various media, and statistics included in the Interactive map for Transitions and Continuity of Care (ITACC) are for informational purposes only. Because the data contained here is constantly evolving, the content of the map may not be accurate, complete, current, or error-free at any particular point in time. It is the responsibility of the user to conduct their own review of the content prior to any implementation. The ITACC map is not a comprehensive guide or standalone document for understanding access to care within our region.
